Orthotropic Treatment (Biobloc)

  • Orthotropic treatment (6 years to 11 years) is an intervention that is focused on guiding optimal craniofacial growth. It results in better facial balance, eliminates dental crowding and dental extractions, and minimizes or eliminates the need for braces or clear aligner therapy during the teen years (“braces age”).
  • Most importantly, this intervention is focused on the patient attached to the teeth rather than the teeth attached to the patient. It means the patient will breathe better and sleep better due to a well-developed airway not constricted from deficient growth (a widespread problem resulting in dental crowding).
  • When a child breathes and sleeps better, they behave better, focus better, and are less hyperactive. Optimal release of growth hormones, metabolic regulation, and cognitive function is optimized.

ALF (Advanced Lightwire Functional)

  • This long-standing and validated form of orthopedic remodeling is a gentle approach to stimulating the growth of the dental arches, especially early in life (less than six years), while working with the child’s natural development. If a child’s deficient craniofacial growth is addressed earlier, the risk of dental crowding and breathing and sleep problems are minimized or eliminated.
  • If the child is in physiologic breathing distress (something that the parent may not recognize), sleep may be a problem, resulting in many growth-related issues.
  • Most importantly, this intervention is focused on the patient attached to the teeth rather than the teeth attached to the patient. Myofunctional Therapy/Myobrace

  • Focuses on tongue function and oral posture that may be compensated due to bottle feeding, sippy cups, mouth breathing, thumb/finger sucking, tongue ties (ankyloglossia), etc. This all results in an increased risk for deficient craniofacial growth and development and often is the root cause of dental crowding later in life and, more importantly, breathing and sleep problems. Myofunctional therapy helps to re-pattern the function of the tongue to optimize oral posture, proper swallowing while eating, dilate the airway and foster optimal growth and development. That is best guided by a licensed sleep & language pathologist, physical therapist, or myofunctional/occupational therapist trained explicitly in myofunctional therapy.
  • Myobrace, Healthy Start, and other branded approaches are excellent adjuncts but not necessarily a substitute for a well-trained therapist.

Maxillo-mandibular Advancement

Maxillo-mandibular advancement surgery, skeletal expanders, & surgically facilitated clear aligner therapy. Are a few of the “tools” in the “tool chest” leveraged to improve/correct breathing and sleep problems such as obstructive sleep apnea—inspiratory flow limitation, upper airway resistance syndrome, sleep fragmentation, CPAP intolerance, etc.

  • Each of these interventions is interdisciplinary, requiring a team approach to assess, diagnose, and execute optimally.
  • Each “tool” can be used in isolation or in addition to other “tools” to optimize outcomes. These tools address structural deficiencies and are implemented in concert with therapy, such as myofunctional therapy, swallowing re-patterning, physical therapy, speech-language and pathology treatment, and mouth, face, and tongue exercises.
  • Each approach varies in risk, benefit, and cost but all offer predictable, positive outcomes when adequately guided by a well-informed and experienced airway team.
