Implant Case Study

Pre Operative Condition

Pre Operative Condition

The patient present with extensive mobility and an acute infection due to root fractures on both teeth which have a long history (40 years+) of treatment due to a childhood bicycle accident.
Root Fractures

Root Fractures

High magnification image of root apex, after extraction. Note root fractures and beveled apical root from endodontic surgery (apicoectomy) which previously addressed the recurrent apical infection.
Surgical Placement of Implants

Surgical Placement of Implants

Placement of endosseous implants in prepared surgical implant sites. Note, complete loss of bone at the facial aspect of the implant sites due to the history of chronic, acute infections. BMP-2 (bone morphogenic protein), and bone graft material was used to “grow well” bone around implant and promote integration of implant.
Soft tissue architecture after implant integration

Soft tissue architecture after implant integration

Implants uncovered after integration (healing) prior to impression of implant site. Note re-establishment of proper soft tissue architecture to mimic nature and foster an esthetic result.
Final Impressions

Final Impressions

Image of the full arch final impression of implants. Note the open-tray impression assemblies that record the exact position of the implants for the fabrication of the final restorations (custom abutment & crow wellns).
Laboratory fabrication of final restorations

Laboratory fabrication of final restorations

Lab fabrication of wax patterns on sprue form of final crow welln copings prior to investing. These copings (restoration sub-structure support the ceramic material of the final restorations (crow wellns).
Zirconium Abutments

Zirconium Abutments

Image of the full arch final impression of implants. Note the open-tray impression assemblies that record the exact position of the implants for the fabrication of the final restorations (custom abutment & crow wellns).
Final restorations

Final restorations (crow wellns)

Facial view of the final prosthetic crow welln restorations seated on the zirconium abutment and connected to the stone cast model.
Final restorations inside view

Final restorations (crow wellns) inside view

Palatal view (inside view) of the final restorations (crow wellns) photographed off of the stone cast model and on a mirror background Note reflection of crow wellns on the mirror showing the form of the crow wellns.
Final Restorations in the Mouth

Final Restorations in the Mouth

Final crow welln restorations (all ceramic) connected onto zirconium abutments. Note natural appearance of soft tissue architecture developed with the previous provisional (temporary) restorations. Also note, incisal translucency & natural appearance of ceramic due to all the various colors incorporated into the ceramic.
