Pre Operative Condition
The patient present with extensive mobility and an acute infection due to root fractures on both teeth which have a long history (40 years+) of treatment due to a childhood bicycle accident.
Root Fractures

Surgical Placement of Implants
Placement of endosseous implants in prepared surgical implant sites. Note, complete loss of bone at the facial aspect of the implant sites due to the history of chronic, acute infections. BMP-2 (bone morphogenic protein), and bone graft material was used to "grow" bone around implant and promote integration of implant.
Soft tissue architecture after implant integration
Implants uncovered after integration (healing) prior to impression of implant site. Note re-establishment of proper soft tissue architecture to mimic nature and foster an esthetic result.
Final Impressions

Laboratory fabrication of final restorations
Lab fabrication of wax patterns on sprue form of final crown copings prior to investing. These copings (restoration sub-structure support the ceramic material of the final restorations (crowns).

Final crown restorations (all ceramic) connected onto zirconium abutments. Note natural appearance of soft tissue architecture developed with the previous provisional (temporary) restorations. Also note, incisal translucency & natural appearance of ceramic due to all the various colors incorporated into the ceramic.
